Suzanne M. Alton, BSN, RNC-OB, Senior Clinical Nurse II

In 1988, I earned my Bachelors of Science degree in Nursing from University of Maryland School of Nursing in Baltimore, Maryland.  I began my nursing career at Johns Hopkins Hospital on a med/surge unit.  In 1989, I left to pursue a career as a Labor and Delivery nurse at University of Maryland Medical Center.  I spent the first 29 years in Labor and Delivery as an Advanced Certified Inpatient Obstetric bedside nurse, charge nurse, Certified Childbirth Educator and led the IVF program on our unit.  In 2018, severe Pudendal Neuralgia pain prevented me from taking care of patients physically, so I had to take a three-month leave of absence.  This and other chronic pain conditions have greatly affected my personal life with my family as well as my professional career.  After my leave of absence, I returned to a new role as the Labor and Delivery Operating Room resource nurse.


Dr. Ashley Gubbels is a board certified and fellowship-trained minimally invasive gynecologic surgeon. She received her undergraduate and medical degrees at the University of Minnesota. She completed her residency in OBGYN at the University of Nebraska Medical Center and then served in the US Air Force as a general OBGYN for 4 years. Upon completion she then pursued a fellowship in Minimally Invasive Gynecologic Surgery at St. Joseph’s Hospital and Medical Center (now under Creighton University School of Medicine) where she trained in the surgical treatment of chronic pelvic pain conditions, including pudendal neuralgia. Following fellowship, she joined the University of Rochester Medical Center’s Center for Pelvic and Vulvar Pain in 2018. She recently returned to Creighton University/St Joseph’s Hospital and Medical Center as a faculty physician in the Advanced Gynecology division.

Dr. Gubbels specializes in chronic pelvic pain conditions such as endometriosis, interstitial cystitis, myofascial pain, and neuropathic pain conditions like pudendal neuralgia. She is well versed in the excisional treatment of endometriosis, peripheral pelvic nerve blocks, and pudendal nerve decompression. She is board certified in OB/GYN, a member of AAGL (formerly American Association of Gynecologic Laparoscopists) and the International Pelvic Pain Society, and on the board of the Pudendal Neuralgia Association.



Tracy Sher PT, MPT, CSCS is the owner of the private practice, Sher Pelvic Health and Healing in Orlando, and founder and CEO of the global platform, Pelvic Guru/Pelvic Global and the Global Pelvic Health Alliance Membership (GPHAM). Orlando Family Magazine named Tracy “Readers’ Choice for Best Doctor for Women 2016” in the area of female pelvic medicine – the only physical therapist on the list. She has specialized training in pelvic physical therapy, strength and conditioning, and sexual health. 

Tracy has a passion for treating complex pelvic health cases relating to pelvic pain, sexual pain, and postpartum changes. As a leader in the area of pelvic health therapy with a biopsychosocial approach, she travels across the US and the world sharing her expertise, teaching courses, and speaking to the community and professionals. She teaches a 2-day course for health professionals entitled Pudendal Neuralgia and Complex Pelvic Pain Solutions. Tracy thrives on being a connector to bring people together and improve the way healthcare providers learn and help others.

Donna Kreher, PhD, is a clinical psychologist dedicated to collaborative care and the advancement of biopsychosocial approaches to pain. Dr. Kreher received her PhD from the University of Massachusetts-Amherst, completed her doctoral internship and post-doctoral fellowship at the University of Rochester Medical Center, and joined the faculty of URMC following completion of her training.

 

Dr. Kreher directs a specialty behavioral health practice within the department of psychiatry, which provides consultation, evaluation, and ongoing therapeutic support for patients with chronic pain. She has a special interest in chronic pelvic pain and has been involved in developing and evaluating new treatment programs for CPP, and in scholarship centered on the relationship between psychological factors and treatment outcomes in CPP. 

 

Additionally, Dr Kreher serves as a member of the teaching faculty for residents in orthopaedic surgery, where she provides in-depth observations and feedback on physician-patient communication skills, professionalism, and team-based care.


Kathryn Witzeman, MD, FACOG is an Associate Professor in the

Department of Obstetrics and Gynecology at the University of

Colorado School of Medicine.  In 2008, Dr. Witzeman founded the

Women’s Integrated Pelvic Health Program at Denver Health

Medical Center and serves as its director. Dr. Witzeman joined

the Board of Directors of the International Pelvic Pain Society in

2014, and currently serves on the Executive Board. She is

board-certified and a Fellow of the American College of OBGYN.

Her clinical and research interests include vulvovaginal disorders,

pelvic pain, menstrual disorders, menopausal problems,

integrative medicine, biomedical ethics as is applies to the field of

OBGYN as well as medical education. She speaks nationally and

internationally on topics of pelvic pain and integrative medicine.

Dr. Witzeman completed her medical degree at the University of

Cincinnati SOM and subsequently completed her OBGYN

residency training at the University of North Carolina, Chapel Hill.

Dr. Witzeman completed the APGO Scholars and Leaders

program in 2012 and graduated from the University of Arizona

Integrative Medicine Fellowship in 2018.


Erin T. Carey, MD MSCR is an Associate Professor of Obstetrics and Gynecology and Division Director of Minimally Invasive Gynecologic Surgery at the University of North Carolina (UNC) School of Medicine. She also serves as the Fellowship Director of Minimally Invasive Gynecologic Surgery. She completed her residency in OBGYN at the Mayo Clinic and then a MIGS fellowship at UNC. While in fellowship she also earned a Masters of Science in Clinical Research (MSCR) at the Gillings School of Public Health and completed an additional year of training in female pelvic pain disorders with pain medicine.

 

She treats the full scope of gynecologic surgical and pelvic pain issues including the management of complex pelvic and vulvar pain disorders. She is co-PI on a R01 designed to evaluate current treatment practices targeting provoked vestibulodynia. As a physician scientist, she strongly believes in collaborative research efforts designed to improve diagnostic and treatment options for women with pelvic pain disorders.



Dr. Rosemarie Filart, MD, MPH is an Integrative Rehabilitation specialist who has been practicing as a physician for over 20 years. Dr. Filart earned her Medical Degree and Masters in Public Health through a dual degree program at George Washington School of Medicine & Public Health. Following medical school, she dual trained in Physical Medicine & Rehabilitation (PM&R) at Baylor College of Medicine, and in Primary Care Medicine at Yale School of Medicine. She has also completed Fellowship Trainings from the Kessler Institute of Rehabilitation and the Academy of Integrative Health and Medicine. Dr. Filart is board-certified in PM&R, Integrative Medicine, Brain Injury, and Spinal Cord Medicine. She is a certified practitioner in Functional Medicine, Acupuncture, Microcurrent and Electrical Stimulation therapies. Recognizing that chronic pain is a complicated condition which often requires a multimodal response, Dr. Filart has experience applying effective, minimally invasive treatments such as various injections, microcurrent/ electrical stimulation and other integrative rehabilitation interventions. Her patient-centered, whole person approach also takes into consideration important lifestyle factors such as nutrition, sleep, stress, activity, and behavioral health.


Howard M. Richard, III, M.D. is an Associate Professor of Diagnostic Radiology and Nuclear Medicine at

the University of Maryland School of Medicine. Dr. Richard is a board-certified interventional radiologist

who specializes in women’s interventions, urological interventions and complex vascular retrievals and

embolizations. In regard to women’s interventions he has a special interest in uterine fibroid

embolization and chronic pelvic venous congestion. In addition he sees patients with chronic pelvic pain

for which he provides diagnostic and therapeutic image guided nerve blocks. In regard to urological

interventions Dr. Richard has extensive experience in urinary stone disease management and

percutaneous image guided cryosurgery surgery for renal cancer. In regard to complex vascular

procedures, he is an institutional resource for patients with difficult embedded IVC filters and the image

guided embolization of persistent endoleaks.

Dr. Richard received his medical degree at the University of Virginia School of Medicine in

Charlottesville, Virginia. He completed a surgical internship at the Johns Hopkins Medical Institutions in

Baltimore, Maryland. He served in the US Army Medical Corps as a General Medical Officer prior to

completing his radiology residency at the Mount Sinai Medical Center in New York City. He completed a

fellowship in interventional radiology at the University of Maryland Medical Center.

Dr. Richard has varied research interests. He is the primary investigator in the Lava trial, a study of a

novel liquid embolic agent at the University of Maryland. He is a co-investigator in the PRESERVE trial

using irreversible electroporation (NanoKnife) in the treatment of prostate cancer. He is also the

principal investigator of an ongoing prospective evaluation of treatments for patients with the pudendal

neuralgia.

 Moderators

Richard P. Marvel, MD, FACOG received his undergraduate degree from Johns Hopkins University in Baltimore where he graduated with honors. He attended the West Virginia School of Medicine and did his Obstetrics and Gynecology residency training at Hahnemann University in Philadelphia. Over the past 20 years, he has been an attending physician at Hahnemann University School of Medicine, Medical College of Pennsylvania, the University Of Maryland School Of Medicine, and a part-time faculty member at The Johns Hopkins University School of Medicine. While an Assistant Professor at these institutions he developed an interest and expertise in the evaluation and management of women with chronic pelvic pain and advanced Minimally Invasive Gynecologic surgery.

He has authored multiple articles and book chapters dealing with many aspects of gynecology and pelvic pain. He is a Fellow of the American College of Obstetrics and Gynecology as well as a Fellow of the American College of Surgery.

He has been directly involved in research and was a member of the Interstitial Cystitis Clinical Trials Group, sponsored by the National Institutes of Health. He is a member of the National Vulvodynia Registry, a multicenter trial enrolling patients with vulvar pain disorders.


He is currently in private practice in Annapolis, Maryland where he has developed the Center for Pelvic Pain at AAMC. He is Director of Minimally Invasive Gynecologic Surgery and Pelvic Pain at the Anne Arundel Medical Center in Annapolis Maryland. Dr. Marvel is a member of the Board of Directors of the International Pelvic Society and Member of the Board of Directors of the Pudendal Neuralgia Association, Inc. He uses an interdisciplinary approach to chronic pelvic pain working closely with physical therapists, psychologists, anesthesiologists, interventional radiologists, general surgeons, and peripheral nerve surgeons, to coordinate a multidisciplinary management of women with pelvic pain. Over 90% of his practice is devoted to managing women with chronic pelvic pain, vulvar pain, and dyspareunia. He is an accomplished laparoscopic surgeon and has extensive experience in the surgical treatment of endometriosis and other causes of pelvic pain. Over the past10 years, he has developed an interest in peripheral nerve causes of pelvic pain and is one of the few surgeons in the United States who performs pudendal, obturator and abdominal wall nerve decompression procedures. He is one of the few clinicians who offers pudendal neuromodulation for the treatment of pudendal neuralgia, bladder and bowel dysfunction, and vulvodynia.


Dr. Jyotsna Nagda received her undergraduate degree and medical degree from Bombay University in India.

She completed her residency training in Anesthesiology at the University of Medicine and Dentistry of New Jersey. She subsequently completed her Pain fellowship at Beth Israel Deaconess Medical Center (BIDMC) / Harvard Medical School, in Boston where she has since been a staff faculty for the last 21 years. She is Board Certified in the Anesthesiology and Pain Medicine. 

She served as the Director of Quality Assurance and Quality Improvement at the BIDMC Arnold Warfield Pain Center and currently holds the position of Pain Fellowship Program Director . She is also a staff member of the BIDMC- Intimate Health and Wellness Center and the Endometriosis center . 

Her clinical and research interests include chronic pelvic pain, spinal and geriatric pain and the simulation program for safety in Pain medicine . She is one of  the founding members and on the board of directors of the Pudendal Neuralgia Association. 

Dr. Nagda also serves on the editorial board of Indian Journal of Pain. She has spoken at numerous national and international venues about topics related to diagnosis and management. of chronic pain conditions including chronic pelvic pain.

She is a member of several professional organizations including the American Society of Anesthesiology, American Society of Regional Anesthesia and Pain Medicine, American Society of Interventional Pain physicians, Spine Interventional Society.

Emotional Aspect of and Coping Mechanisms for Pelvic Pain Webinar with Nancy Fish, MSW, MPH, LSCW

Nancy Fish, MSW, MPH, LSCW received her masters’ degrees in social work and public health from Columbia University in 1984. She has a full-time private practice in Bergen County New Jersey (also does skype sessions with individuals unable to come to her office).  Nancy uses an eclectic approach combining psychodynamic, family life cycle, mindfulness, and cognitive behavioral techniques.  Nancy specializes in working with men and women suffering from sexual pain. She also works with couples, families, individuals, and adolescents and deals with a range of issues including bereavement, depression, anger management, and anxiety. After developing sexual pain issues in 2006 and being treated by Dr. Coady, Nancy felt it was her mission to help women suffering from these horrendous and debilitating conditions. Nancy provides a unique insight and perspective into her patient’s issues. She is able to offer deep understanding of these women’s emotions and due to the successful management of her condition, can offer women hope and insight in how to cope with the overwhelming task of recovering from sexual pain conditions. Nancy co-authored with Dr. Deborah Coady the book, Healing Painful Sex: A Woman’s Guide to Confronting, Diagnosing and Treating Sexual Pain.

The presentation will focus on the many emotional aspects of having pelvic pain including depression, anxiety, relationship strain, employment issues and altered sense of self and sexuality. She will also focus on the many coping mechanisms to help manage your life life and pain including cognitive behavioral techniques, mindfulness, and integrative treatment.

She is a licensed professional counselor in Pennsylvania and is a Master Practitioner of Neuro-Linguistic Programming, Emotional Freedom Techniques (EFT) and Mindfulness-Based Stress Reduction Trainer. She works with male and female patients suffering from chronic pelvic and sexual pain, providing counseling, education and mind/body relaxation techniques for stress and pain management. Her private counseling practice specializes in chronic pelvic and genital pain, trauma and sexual health. In this webinar, she will explain the neurobiology of pain and trauma and how mindfulness techniques have been shown to help heal this damage within the brain and nervous system.
